New Students

New Students must complete the Online Application and all required forms. 
Complete and return each of the following:
     Health Inventory Form
     Medication Authorization Form
     Epipen Administration Form
     Maryland Immunization Form
     Emergency Contacts Form
     Permissions Form

 - or - 

Returning Students
Returning students must logon through the Family Portal to re-enroll student(s).

Complete and return all of the following:
 Health Inventory Form
 Medication Authorization Form
 Epipen Administration Form
 Maryland Immunization Form
 Emergency Contact Form
 Permissions Form
 Student Pledge of Cooperation (Kindergarten - 12th only)
